The Process

Once you’ve made the decision to communicate with a ReMend Mentor, you can complete a referral here or reach out to your social worker at your dialysis unit, your social worker will send a referral to ReMend. 

A ReMend Intake Specialist will contact you and ask a few questions in order to match the right mentor to meet your needs.

Once a mentor has been selected, you’ll be contacted by a mentor within 48 hours. You have several options when communicating with your mentor:  by phone or online via Skype, meeting up at your dialysis clinic or a mutually agreed upon location, like a local coffee shop. We want to make it easy for you!

Paying it forward

As part of the beginnings of ReMend, my friend and Mentor; Dennis Bourque’s conviction and passion for paying it forward with respect to helping others with serious kidney diseases was a no-brainer for me.

Fortunate to have two very good friends willing to sacrifice one of their kidneys is a blessing and being a member of ReMend has allowed me to help others to provide needed answers and direction in handling their kidney disease. Being a member of ReMend has been a true blessing in my life.
